You recieve name change letters, faxes and forms in your name for your selected organisations.  Customers purchasing the DIY kit need to write their own letters, so it only recommended for clients with fewer than 6 organisations to contact.

Our DIY Kit provides step by step instructions to guide you through the entire name change process, including an easy to follow checklist, tips & any additional information you need to help you along your way.

You'll receive the most common forms that you'll need to have processed, including driver license forms (if required in your state), Medicare and more.  You’ll also get the forms to apply for your marriage certificate or legal name change certificate (note there are separate fees payable to the Registry of Births, Deaths and Marriages).

We provide you with all the information you need to you can be confident in managing the process yourself. The DIY kit includes contact details for popular Australian organisations so you can quickly find out what supporting documentation you need & where to send your information to. We provide you with a letter and fax template as there will be several companies that will need you to send a letter, however you will need to write these letters & address the envelopes yourself.

Please note

Easy Name Change kits do not include any certificates from the Registry of Births, Deaths and Marriages (BDM).  You will need to obtain your marriage, birth or name change certificate from the Registry in the state where your event took place.  There is a separate fee payable directly to RBDM.
