If you’re an ANZ Super Advantage policyholder who has recently changed names, you’ll need to get the name on your super policy updated. Complete the ANZ Super Advantage Change of Member Details form, included in your Easy Name Change kit. You can also call ANZ Super and ask them to mail you a copy of the form.
