Spirit Super name change process

We provide the Spirit Super change of name form, which is personalised and ready to send. You’ll need to provide a certified copy of updated photo ID, so it’s a good idea to prioritise your driver’s licence or non-driver photo ID card. If you haven’t updated your photo ID yet, you can instead send your proof of name change document (eg: a marriage or legal name change certificate from Births, Deaths and Marraiges) along with photo ID in your former name.

Superannuation funds always require certified copies because they hold such a large asset on your behalf, so it’s important they safeguard your account and prove your identity.
