Superannuation Name Change Procedure

If you’re a member of the Government Employees Superannuation Board (GESB), you should update your policy soon after changing names. If you hold multiple super policies, you may want to roll over all your policies into one fund before changing names, then you won’t need to provide identity again from the names on your policies not matching.

GESB members should complete the GESB Change of Details form, included in your Easy Name Change kit. Return the form using the pre-addressed Easy Name Change mailing slip. You’ll also need to include certified documents, as listed on the form. Your name is generally changed within a few days of your form being received.
