If you’ve changed names ensure you update your Aware Super policy. If you have more than one superannuation policy, it’s a good idea to roll over all your policies into one account before changing names, otherwise, you may be having to prove your identity again if the name on your super policies does not match.
Complete the Aware Super name change form. This form is included in your Easy Name Change kit, or else contact Aware Super and ask they send you a copy of the form. Provide your old and new name details. You can also use this form to notify of a change of address or to provide your Tax File Number. Sign the form and return using the pre-addressed Easy Name Change mailing slip. Both the Aware Super form and the Easy Name Change mailing slip explain what proof of name change and identification documents must be included.
