Superannuation Name Change Procedure

When changing names from marriage, divorce or legal, it is always important to keep your superannuation details updated. It’s also a good opportunity to roll all other policies into the one account, as it can be difficult to prove your identity when you have multiple accounts under different names,

State Super customers must complete the Change of Personal and Banking Details form. This is provided in your Easy Name Change kit, request the form from HR, or else call State Super and ask the form to be mailed to you. Complete the form, providing your full old and new names. You’ll also need to provide certified copies of both your government-issued photo ID and your name change document.
