Customers need to visit a First National Bank location in person with their original proof of name change and updated photo ID.
If you’ve changed names from marriage, divorce or court order then you’ll need to start by visiting these 3 places in person:
- Start with filing the SSA name change form in person. While you are able to send the form by mail, it’s not recommended as you need to provide original proof documents. We provide the SSA name change form in our kit, or else you can get it directly from the SSA.
- Allow 2-3 days for the SSA name change to filter through their system, then visit your DMV in person. All states require you to go into a branch so that they can sight your original documents and your signature can be witnessed. If your state has a special form, we provide it.
- First National Bank of Omaha. Most banks require you to visit in person so they can verify your identity.
