If you’re a KeyBank customer and have changed names for any reason, visit a branch in person. Bank staff will update the name on your account, witness your new signature and request replacement checkbooks and bank plastic cards. Be sure to take your original proof of name change (eg: marriage certificate, divorce order or legal name change order). You also need an updated photo ID.
Change names with the SSA first
Send in the SSA name change form, which is included in your kit, and wait for proof your name has been updated. Next, attend your state DMV in person to get an updated driver’s license or non-driver ID card. Your state DMV may have a form. When you attend the DMV you may be required to pay a small fee for a duplicate license and have a new photo taken.
