Via Credit Union customers must attend a branch in person with the required documents. As with all name changes, start by changing your name with the SSA. We provide the SSA name change form in your kit.
Once you have confirmation that the SSA has made the change you can attend Indiana BMV to get an updated driver’s license or non-driver state ID card. You may need to pay a license replacement fee.
