Where do I start?
As with all name changes, follow this process for a smooth name change:
- File the SSA name change form in person. It is provided in your kit. You can file by mail but you’ll need to send original identification and it can take a while to be returned.
- Attend the DMV in person. We provide your state form if they have one. Check our DMV instructions for your state’s duplicate fees, what additional proof documents may be required and see if there’s a link to book your appointment online.
- Most banks and credit unions need to be visited in person. Take your original proof of name change and updated photo ID. Read the instructions for your bank as some have forms or may need proof of new name with SSA.
- Send off your Easy Name Change forms, letters and emails.
