Great Lakes make it easy to change names! Simply send in the Easy Name Change letter with the proof documents listed at the bottom. If you chose to receive ready to forward emails instead of letters, then just click ‘forward’ on our email, attach the necessary proof and send!
As you need to provide updated photo ID, be sure to choose the name change form for the SSA. Sign the form and send in with the proof listed. Once the SSA confirms your name has been changed attend your state DMV to update your driver’s license or non-driver ID card. If your state DMV has a special form we also provide that for you!
